What was the specific need/connection to the service plan? Aunt, who lives in Portland, advocates on behalf of her 11 year old nephew, who lives in Linn County. The family has experienced barriers around IEP support in the school as well as barriers around community supports. Youth is currently with Grandmother (permanent guardianship). Aunt identified that finding support for Grandma will be most helpful for the family right now as well as information on upcoming CPS Parent Groups (morning time would be best, if available).
What support did you provide? I listened actively, offering words of encouragement and identifying strengths. I offered space for Aunt to share her her experience nagigating on behalf of her nephew. Aunt identified some resource information that might be helpful for the family. I offered to gather information and send via email, at Aunt's direction.
What are next steps? With Aunt's permission, I will call to check in on 3.10.21, follow up with resource information via email, Lindsay will offer to sit alongside Grandma to connect with warmline
